The quest "For Whom the Bell Tolls" kicks off immediately after you finish "Wedding Crashers." You find yourself imprisoned in Trosky, where Hans faces execution once the bell tolls 12 times. Fortunately, Henry is spared from the dungeon and assigned to labor around the castle as a form of punishment. Seize this fleeting freedom to orchestrate Hans's rescue.
Hauling Sacks

Begin your quest by hauling sacks to a shed. This task is mandatory, so grab a sack from the wagon and transport it to the shed adjacent to the fenced area. Deposit the sack inside the shed and repeat the process until you've fulfilled the objective.
Help the Blacksmith

Next, assist the blacksmith, who, despite his reluctance, assigns you to craft a horseshoe. Head through the archway opposite the blacksmith to reach the forge area. Gather the necessary materials from the forge and create the horseshoe. Return to the blacksmith for his approval, then proceed along the path you used for hauling sacks to locate the stables and deliver the horseshoe.
Help Fanka the Cook

After delivering the horseshoe, shift your focus to aiding Hans. Return to the forge and retrieve a lockpick from the red pot on the shelves. Engage with the woman nearby, Fanka, a cook in the noble's kitchen, who you wouldn't normally be allowed to access. Offer your help to Fanka to gain entry.
Accompany Fanka to the kitchen. While lockpicking isn't necessary, doing so boosts your reputation, and Fanka offers you the privilege of accessing food whenever needed—a perk that could prove invaluable in the subsequent stages.
Speak with the Chamberlain

Upon entering the kitchen, you'll notice the chamberlain suffering from pain. Engage him in conversation and attempt to persuade him to allow you to concoct a Digestive Potion. Success grants you access to the surgeon's workshop; failure means you'll need to sneak in.

If you couldn't persuade the chamberlain, gather some food from the kitchen to facilitate your stealthy entry into the workshop. Otherwise, proceed with the granted access. Navigate to the stairs opposite the forge and enter the kitchen used by non-nobles within the castle. A guard awaits here. If sneaking in, offer him the kitchen food to prompt his departure, allowing you to ascend the stairs undetected.
Ascend multiple flights of stairs until you reach a corridor with two doors. Enter the surgeon's workshop through the right door. Craft your Fever Tonicum here, and if you convinced the chamberlain, also prepare a Digestive Potion. The room's chest contains all the required ingredients for both concoctions.
Cure Thomas

If assisting the chamberlain, return to him with the Digestive Potion to gain access to Thomas's room. For those sneaking, another route awaits before quest completion.
Return to the noble's kitchen and ascend its stairs to evade additional guards rather than crossing the bridge between the workshop and kitchen. Maintain a crouched stance as you climb. You'll encounter a guard at the base of a staircase leading to Thomas's room.
Distract the guard with a thrown pebble, prompting him to leave his post, then stealthily ascend the stairs. Continue until you reach Thomas's room, where he lies ill with his sister, Adele, attending to him. Hand over the Fever Tonicum to Adele, triggering the cutscene that concludes this quest.
